      How to Fill up an EPF Form 11

      Employees need to fill in the following info.

      • Employees’ name
      • Name of father or spouse
      • Gender 
      • DOB
      • E-mail ID and mobile number
      • Marriage Status

      Information Linked to Former Employers and EPS and EPF Involvement

      • If employees are or are not current members of the EPF Scheme, 1952.
      • Whether individuals are current members of the EPS Scheme, 1995.
      • If employees have shown they want to engage in any of the 2 schemes, the following information must be provided
      • UAN.
      • Former Provident Fund or PF Account Number
      • Date of departure from the prior employment in the date-date/month-month/year-year-year-year style
      • Certificate of Scheme Number if got issued
      • If a PPO got issued

      • Passport No.

      Know Your Customer Info

      Employees’ Undertaking

      Submission of EPF Form 11

      Once employees have finished filling out EPF Form 11, they must send it to their employer. The employer will sign and stamp the paperwork. The form will then be sent to the EPF office in the region by him. If employees have a UAN, the transfer will be significantly easier, and the transaction may be completed online.

      Employer’s Obligations?

      • Ensure that all new joiners fill out the form of declaration within a month and post the data on the UAN site in 25 or twenty-five days of the conclusion of every month.
      • Communicate the UAN details obtained by Employee provident Fund Organisation. with all current fund members within 15 days of receiving the UAN and get their confirmation regarding the same.
      • Within one month after receiving the UAN, update the KYC data inclusive of bank account info, Aadhaar Card, and PAN of those employees.
      • If the members do not possess Aadhaar cards, the employer needs to provide the Aadhaar Acknowledgement Slip within 30 days of getting the UAN.
      • If employers obtain Aadhaar info from employees, they need to give the info to the website of UAN 15 days after getting it.
      • The claim form should be thoroughly verified by the employers before being sent to the Employee Provident Fund Organisation from the employer’s end. The employer should ensure that it is comprehensive like linking the required KYC data with UAN.
