An individual might name one or more family members to receive the provident fund earnings following his or her death. Employee Provident Fund Scheme 1952 defines ‘family’ as husband/wife, kids (married/unmarried, adopted), and dependant parents. According to the Employees Pension Scheme 1995, the family consists of the husband/wife and their children. If an employee desires to modify the names of his or her nominees under the Employees Pension Scheme 1995, he or she must submit an updated Form 2 to the employer.

EPF Nomination Form Types
The following are the three types of nomination forms:
- PF Form 2: This is a popular form (used in pension and PF systems) for distributing fund amounts to candidates.
- PF Form 11: This form is a member’s statement that he or she works in a factory or firm that is covered by both the Employees’ Family Pension Fund Scheme and the Provident Fund.
- PF Form 9: This form is used by an individual who is starting employment in a plant or facility where the Employees’ Pension Scheme is in effect.
PF Form 2 Information Requirements
PF Form 2 is a nomination and declaration form under the Employees Pension and Provident Fund schemes of the Employees Provident Fund Scheme 1952.
A member must fill out the following information on EPF Form 2:
- The member’s name
- Name of the father or husband
- Permanent address
- Date of birth
- The nominee’s name
- The nominee’s complete address
- Relationship between the member and the nominee
- The nominee’s birth date
- Each candidate will receive a portion of the PF amount.
- If the nominee is a minor, the member must provide information on the custodian who will earn the PF amount.
- If the member does not have any dependent parents or relatives as defined in paragraph 2 (g) of the Employees Provident Fund Scheme 1952, he shall delete the portions that are not relevant.

- Information of family members who will get the children’s or pension widow’s if the member passes away. The member must provide the names, ages, and addresses of all family members. The member should also indicate their connection to the nominees.

PF Form 11 Information Requirements
EPF Form 11 is a nomination and declaration form required under Section 24 of the Employees Pension Scheme (EPS) of 1995, as well as Sections 34 and 57 of the Employees Provident Fund Scheme of 1952. A member must include the following information on PF Form 11:
- The member’s name
- The member’s birthdate
- Name of the father or husband
- Email Address
- Mobile Number
- Individuals must state whether or not they were a member of the Employees Provident Fund Scheme in 1952 or the Employees’ Pension Scheme in 1995. If yes, the individual must fill out the following information about his or her former employment: UAN or prior provident fund account number, Exit date, Previous scheme certificate number, and Order for previous pension payments
- International employees must include the following information: Origin Country, Passport number Validity of passport, Qualifications in education, Any disability or marital status (visual, locomotive, hearing)
- The following KYC information must be included: IFSC code for a bank account (mandatory), Aadhar Card, PAN, Passport, Driving License, Voter ID, ESIC Card, and Ration Card.
All of the above documents must be self-attested and that includes the current employer’s declaration and the employer’s signature and the establishment’s seal.
PF Form 9 Information Requirements
EPF Form 9 is a declaration form for the Employees Pension Scheme (1995) and the Employees Provident Fund Scheme (1952). (para 34 and 57). This form is a statement by a member who is starting work in an institution that is covered by both the Employees’ Provident Fund Scheme of 1952 and the Employees’ Pension Scheme of 1995. Form 9 asks members to provide the following information:
- Date of birth of the member
- Name of the member’s father or husband

- Mobile Number
- Individuals must state whether or not they were a member of the Employees Provident Fund Scheme in 1952 or the Employees’ Pension Scheme in 1995. EPF E-Nomination Prerequisites
- Make sure your gateway profile photo is up to date. You will be unable to amend your EPF nominee online if you do not do so. The same is true for marriage status, permanent and current residences, and birth dates.
- Your name, gender, and date of birth on EPF records should match those on your Aadhaar card.
- Important details regarding the nominee, such as their name, portrait, date of birth, and Aadhaar card number, are required for EPF E-Nomination.
- You should integrate your Aadhaar card and cellphone number.
- Your brothers and sisters are not considered family members under PF legislation.
