    What is EPF Form 5? EPF Form 5 is for First-Time Employee Registration. Each month, employers fill out EPF Form 5 with information on fresh hires from the previous month. These fresh hires qualify for the services of EPF. 

    Employee Provident Fund Organization applies to all companies in the organized sector industry with 20 and more workers. These companies must offer EPF facilities to qualified employees as well as deposit to their EDLI, EPS, and EPF.     Each month, employers fill out EPF Form 5 with information on fresh hires from the previous month. These fresh hires qualify for the services of EPF. Any employer submits a return for workers who become eligible for participation in the EDLI, EPS, and EPF for the very first time in that one given month.     • Firm address and name
    • The number code of the firm
    • Table of Info of Employee
    1. Serial No. of employee
    2. Info of Account No. 
    3. Employee’s name
    4. Name of father or Name of husband for women who are married
    5. DOB
    6. Sex
    7. Specific date of admission to EDLI, EPS, and EPF
    8. Maximum prior service time at the dates of entering the Fund. Must also enclose Scheme Certificate if possess any.

    What is EPF Form 5 FAQs

    According to the rules, when you are the employer, you must notify the Employee Provident Fund Organization of any new hires in your establishment/organization in order for them to be assigned a UAN. If employees join your company within a month, you must notify Employee Provident Fund Organization by 15th of the following month.
